网上申请新鹰卡/上传照片

应用 for your new 鹰牌 online 和 upload your photo. 没有初始成本为您的卡.

UHCL underst和s that individuals may choose to identify themselves with a preferred 与法定名称不同的名称. 鹰卡持有者可以显示 preferred name they want to be addressed by, regardless of whether they have legally 改了名字. A preferred name is defined as an alternative to the individual's legal name as designated by the individual in university systems. 一个人的法律 name is the name they use for official governmental documents, such as licenses, passports, 还有纳税表格.

The university has the discretion 和 reserves the right to remove a preferred name that is vulgar, offensive, obscene or fanciful, or creates confusion of the individual 和另一个人. The use of a preferred name cannot be used for the purpose of misrepresentation, 和 does not constitute legal 识别. 获发的人士 cards with a preferred name that is different from their legal names should be cautioned that such cards are issued for the purpose of university business only, 和 that campus access cards issued by the University of Houston-Clear Lake are not considered government-issued 识别.

卡丢失或被盗

If you have lost or damaged your card, or if you believe it's been stolen, go to 鹰卡在线. Under "Account 管理ment," select "卡丢失或被盗" to deactivate the card. 然后选择“禁用." If the card is found, you may reactivate your card through the 同样的过程，选择“激活”."

Students may receive a 鹰牌 after making a partial or full payment of tuition 和费用.
Students with "Pending" financial aid are eligible to receive 鹰牌.
A valid government-issued photo ID must be presented in order to pick up 鹰牌.
Continuing students must return former university ID card, in order to receive new 鹰牌.
Student employees may request building/room access from their hiring department.

教师和 staff members may receive their 教师/工作人员鹰牌, provided they have an active record in PeopleSoft 和 the 鹰牌 system. 教职员工和附属机构 will have card labels that reflect their role in the PeopleSoft system. 教师、工作人员和 affiliates wishing to confirm eligibility may contact their hiring department 或人力资源(B2537, x2160).
An eligible employee enrolled as a UHCL student will have the "教师/工作人员" label 印在鹰卡上.
Employees must return former university ID card, in order to receive new 鹰牌.
A valid government issued photo ID must be presented in order to pick up your Hawk 卡. 信用卡订单不需要带照片的身份证.
